Early Childhood Program Welcome Letter

Welcome, New Families! We are looking forward to seeing your children and you on campus this fall. During the summer, you will continue to receive information about the start of school. In the meantime, we wanted to share information with you regarding some of the required forms we will be collecting.

Required Forms

We use the platform Magnus Health to securely upload all required medical and child registration information. You will receive an email from Magnus when your account has been set up for the 2026-27 school year. So that you can start to gather what you will need, enclosed in this folder are some of these forms.   

Child Registration form (upload to Magnus and bring the hard copy to the school)

  • Please make sure that all lines are filled out, complete with addresses and phone numbers. 
  • At least one emergency contact must be within 1 hour of the School.

Virginia School Entrance Form

  • The first page is completed by parents, and the remainder is completed by your child’s doctor. 

ECP staff will be on campus in August, and you may drop off any remaining forms between the hours of 8:30 a.m. and 4 p.m. We will need to see your child’s original birth certificate or passport in person, no later than Aug. 17. Please note that students will not be permitted to start school until all required forms have been submitted. 

For students with allergies or asthma, you will need the forms below. Please see below for instructions on completing them.

Magnus Health Form Instructions

  • Section A filled out by parents
    • Fill in all blanks, listing St. Anne’s-Belfield School as the Child Care provider.
    • Medication name can be specific or the generic name (sometimes doctors order an Epi pen one time and Auvi-Q the next, this would allow us to administer either)
    • Start and End Date should be listed as one calendar year, as this form is resubmitted each year.
    • Please sign and date the page
  • Section B filled out by physician
    • All of the blanks need to be filled in  
    • The physician will need to sign and date the page.
    • Please be sure the information for dosage and times to be administered is the same for both sections.

  • Completed by parents and signed by the physician 
  • Please be sure the Medication name and dosing is the same as the Medication Authorization form
  • The top portion is completed by the parent/guardian.
  • Everything below the gray line is completed by the physician, signed by the parent/guardian, and the physician. 

All emergency medications must be provided unopened, in the original container with the prescription attached.
